Other plans offer lower rates but charge a higher copay with a lower allowance that will only cover either frames or contacts. Heritage offers a premium plan with a slightly higher monthly rate, but charges only a $5 copay. Plus, our annual allowance is double that of other plans, and we don’t ask you to choose between eyewear or contacts.

  • Will I be able to find a provider?

  •  How much does the individual plan cost?
    Premiums are 14.58/mo. Copayments are $5 for eye exams when using an in-network provider, and $10 for frames and lenses.
  •  What does vision insurance cover?
    In any twelve-month period, our individual vision plan will cover an eye exam, one pair of standard frames and lenses, and contacts.
  • Who is covered by my vision insurance?
    Our individual insurance plans are for adults aged 19 or older.
  • Who can buy this plan?
    Our partnership with Blue Cross Blue Shield of Michigan means that any adult currently living in any Michigan county is eligible to sign up.
  • When is open enrollment?
    For individual insurance plans, enrollment is all year! Enroll by the 15th of any calendar month to receive vision coverage starting on the 1st of the following month. So if you enroll by December 15th, coverage will start on January 1st. If you enroll by January 15th, coverage will start by February 1st.
  • What is open enrollment?
    Open enrollment is a window of opportunity where members can sign up for vision coverage or make changes to their current plan.
