QUESTION
What out-of-pocket expenses will my employees incur if
Heritage administers our vision benefit plan?
ANSWER
As long as employees use a network
provider and obtain products and services covered by the
plan, they will not pay any out-of-pocket expenses. If
they visit a provider outside the Heritage network, they
will pay the difference between the cost of the service
or product and the contract-specified out-of-network reimbursement
rate.
